What is a breast reconstruction?

A breast reconstruction restores the shape of the breast after a partial or total mastectomy. The goal is a natural, balanced silhouette, so that you feel comfortable in your body and clothes again. The reconstruction can be performed using autologous tissue or a breast implant and often takes place in several steps.

When is a breast reconstruction considered?

A reconstruction is possible after a total mastectomy, after a breast-conserving procedure with loss of shape, or as part of a preventive mastectomy in cases of a significantly increased hereditary risk, such as a BRCA gene mutation. Highly asymmetrical or congenital abnormal breast development can also be a reason.

The reconstruction can take place immediately during the mastectomy (immediate reconstruction) or at a later time (delayed reconstruction). Which approach is most suitable depends on the breast cancer treatment, any radiation therapy, your general health, and your personal preference. This is always determined in consultation with the breast team.

Reconstruction with autologous tissue or with an implant

In a reconstruction with autologous tissue, skin and fatty tissue from another location, usually the lower abdomen, are used to form a new, natural-feeling breast. The most refined technique for this is the DIEP flap (also called DIEAP flap), where only skin, fat, and blood vessels are moved, sparing the abdominal muscles. This microsurgical technique provides a lasting, natural result that evolves with your body.

In a reconstruction with an implant, the breast shape is built up with a breast prosthesis, often after an intermediate step with a tissue expander that gradually stretches the skin. This approach is technically simpler and involves a shorter recovery, but generally feels less natural than a reconstruction with autologous tissue.

In some situations, both techniques are combined, or the result is refined with lipofilling, using your own fat. During the consultation, we will discuss which technique best fits your situation and expectations.

The process and recovery

A breast reconstruction often takes place in several steps, spread over several months. After the initial breast construction, additional procedures may follow, such as balancing the other breast, nipple reconstruction, and applying an areola tattoo.

The length of the hospital stay and recovery depends on the chosen technique. A reconstruction with autologous tissue requires a stay of several days and a recovery period of several weeks; an implant reconstruction typically has a shorter recovery. You will receive clear guidelines after the procedure and will be closely monitored.

Reimbursement

In Belgium, breast reconstruction after a mastectomy due to breast cancer is reimbursed by health insurance. This also includes balancing the other breast and nipple reconstruction. You will receive clear and complete information about this during the consultation.

When can a breast reconstruction take place?

What is the difference between autologous tissue and an implant?

With autologous tissue, the breast is built up using your own skin and fat, providing a natural and permanent result. With an implant, this is done with a breast prosthesis: a technically simpler procedure with a shorter recovery, but a less natural feel.

Does a reconstructed breast feel natural?

A reconstruction with autologous tissue generally feels soft and warm, comparable to a natural breast. However, sensation in the skin only partially recovers. With an implant, the shape can be well restored, but the breast feels somewhat firmer.

How many procedures are required?

A full reconstruction usually takes place in several steps: first the construction of the breast, then any adjustments, balancing the other breast, and finally the nipple reconstruction. The exact number depends on your situation.

Is reconstruction possible after radiation therapy?

Yes. After radiation, a reconstruction with autologous tissue is often the most recommended choice, as irradiated skin is less suitable for an implant. This will be discussed with you during the consultation.
