The difference lies in what happens next, and that difference determines which concern each technique is suitable for.

Microneedling: stimulation at the surface

Microneedling uses fine needles to create tiny channels in the skin. These micro-injuries are not visible to the naked eye, but the body responds to them as it does to any injury: with a repair process in which new collagen and elastin are produced.

The depth is adjustable but remains within the upper layers of the skin. This makes the technique excellent for texture: uneven skin, fine lines, enlarged pores, and a dull complexion. It is also used to deliver active ingredients deeper into the skin.

After a treatment, the skin remains red for a few days, comparable to a mild sunburn. Most people resume their activities the following day.

RF microneedling: stimulation in the depths

RF microneedling adds radiofrequency energy. Heat is delivered via the needles at an adjustable depth within the skin. This heat causes existing collagen fibers to contract while simultaneously initiating a longer-term repair process in the deeper layers.

Because the energy is delivered at depth rather than at the surface, the upper layer of the skin is relatively spared. This makes the technique better suited for higher skin types than techniques that act on the skin surface, although caution remains necessary.

The scope is broader: laxity, deeper acne scars, and a fading jawline. The recovery time is also somewhat longer, typically involving a few days of redness and sometimes slightly swollen skin.

Which technique for which concern?

Dull skin, enlarged pores, fine lines: microneedling is usually sufficient.

Superficial acne scars: microneedling, possibly in combination with PRF.

Deeper boxcar or rolling scars: RF microneedling.

Early-stage laxity: RF microneedling, possibly combined with biostimulators.

Significant skin excess or sagging: neither. See skin tightening without surgery for where that limit lies.

What both techniques have in common

Both work through the skin’s own healing capacity. This means that results build up slowly: collagen needs weeks to months to mature. Anyone expecting a difference after a single session will be disappointed.

Both also require multiple sessions, usually three to four, with four to six weeks in between. And for both, sun protection in the following weeks is not a recommendation but a requirement: treated skin exposed to the sun without protection can become discolored.

When neither is indicated

In cases of active, inflamed acne or a skin infection in the treatment area.

In the case of a tendency toward keloid formation.

During pregnancy and breastfeeding.

Following recent treatment with isotretinoin, depending on the dose and timeframe.

In the case of certain coagulation disorders or use of blood thinners, after consultation.

When expectations do not align with what the technique can offer.

Frequently asked questions

Is RF microneedling always better?

No. When the problem resides in the upper layers of the skin, the heat adds little value and only results in more strain and longer recovery time. The right technique is the one that matches the depth of the problem.

Can I combine the two?

This is possible within a treatment plan, but not in the same session. Usually, the technique that suits the primary concern is chosen, and adjustments are made based on how the skin responds.

Can I do this at home with a dermaroller?

A device for home use does not work at the same depth and offers no control over sterility or settings. The risk of infection and discoloration is real, and with scar tissue, incorrect application can worsen the appearance.

How long does the effect last?

Both techniques require maintenance, typically one to two sessions per year. After all, the skin continues to age even after treatment.

This text is general in nature and does not replace personal medical advice.
