Both are injected, both provide volume, and both are promoted as a way to make a face look fresher. However, they work in fundamentally different ways, and this difference determines which product is appropriate for a given situation.

A filler provides volume

A filler based on hyaluronic acid applies a gel to areas where volume is lacking. Hyaluronic acid is a substance that also occurs naturally in the skin and binds water. The effect is immediate: the volume that is injected is the volume you see.

This makes fillers predictable and manageable. It also makes them reversible: hyaluronic acid can be dissolved with the enzyme hyaluronidase if the result is unsatisfactory or if a complication occurs. This reversibility is one of the most important safety features of this type of product.

The body gradually breaks down hyaluronic acid. Depending on the area and the product, the effect typically lasts six to eighteen months.

A biostimulator encourages natural production

Biostimulators work in the opposite way. The injected product is not intended as a filler, but as a stimulus. It triggers a controlled reaction in which the fibroblasts in the skin are encouraged to produce their own collagen. The product itself is broken down in the process; what remains is tissue that the body has built itself.

This explains why little is visible immediately after the treatment, and why a change only builds up over two to three months. It also explains why the effect generally lasts longer: natural collagen is broken down more slowly than a gel.

The downside is that a biostimulator is not dissolvable. There is no enzyme that reverses the reaction. For this reason, they are handled more cautiously, in smaller quantities and with more attention to the indication.

When to use what?

A clearly defined volume deficit — for example, a furrow or a specific area that needs support — is suitable for a filler, because the result is immediately manageable.

A general decrease in firmness and density over a larger area is better suited for a biostimulator, because it is not about a single location but about the quality of the tissue.

A face where both issues occur — which is most common — often requires a combination, where the biostimulator provides the foundation and the filler handles the fine-tuning.

Pronounced sagging requires neither. In such cases, a surgical correction such as a facelift is the recommended path; injections then make the appearance heavier instead of fresher.

The pitfall of filling

The most common mistake in aesthetic medicine is not an incorrectly performed injection, but an incorrect indication. When sagging is treated as if it were volume loss, volume is added to tissue that is already too heavy. The face then becomes fuller, wider, and rounder — and looks “done.”

Furthermore, this process occurs slowly and in stages, making it difficult for the individual to notice. For this reason, during a consultation, the mechanism is identified first and only then is a technique chosen, with a consciously conservative dosage and a follow-up appointment.

What about autologous alternatives?

In addition to both, there is a third way: working with material from your own body. Biofiller is prepared from your own blood components and addresses both volume and skin quality. Polynucleotides focus specifically on the quality and hydration of the skin without adding volume.

Which of these paths is recommended in your situation depends on exactly what has changed and what you want to achieve. You can find an overview of the full range of options on the Rejuvenation page.

Frequently asked questions

Is a biostimulator more natural than a filler?

The result comes from your own collagen, which is often perceived as more natural. However, the product itself is equally foreign to the body. A natural look is primarily the result of a correct indication and a conservative dosage, not the product label.

Can I combine both?

Yes, and this happens regularly. They are typically not administered on the same day or in the same area, so that the effect of each can be assessed separately.

How many sessions are needed?

For a filler, one treatment with a follow-up is usually sufficient. For a biostimulator, work is often done in two to three sessions, with several weeks in between.

What happens if I stop?

In both cases, the situation gradually returns to its natural course. There is no question of accelerated aging after stopping the treatments.

This text is general in nature and does not replace personal medical advice.
