This text provides a factual comparison of what each technique can and cannot achieve, as well as the considerations discussed during a consultation. <\/p><p class=\"wp-block-paragraph\">Breast augmentation with fat transfer or with implants is not a matter of one technique replacing the other. They are two methods with different operating principles, different scales of results, and different long-term progressions. <\/p><h2 class=\"wp-block-heading\">Two paths to more volume<\/h2><p class=\"wp-block-paragraph\">Adding volume to the breast can be done in two ways. Either a silicone implant is placed, or the body&#8217;s own fat tissue is harvested from another part of the body and transferred to the breast in small quantities. This second technique is called lipofilling.  <\/p><p class=\"wp-block-paragraph\">The difference is fundamental. An implant is a pre-determined volume that shapes the breast in a controlled manner. Lipofilling works with living tissue that must graft to its surroundings and establish its own blood supply. Consequently, the result of lipofilling is less predictable in terms of size, but it is entirely autologous.   <\/p><h2 class=\"wp-block-heading\">How lipofilling works<\/h2><p class=\"wp-block-paragraph\">In <a href=\"https:\/\/www.drcolle.be\/en\/dienst\/lipofilling-of-the-breast\/\">breast lipofilling<\/a>, fat tissue is harvested via liposuction, usually from the abdomen, flanks, or thighs. This fat is filtered and purified, after which it is applied to the breast in fine layers through small incisions. The technique works with many small quantities in different tissue layers, ensuring each fat globule makes contact with well-vascularized tissue.  <\/p><p class=\"wp-block-paragraph\">Two conditions determine whether lipofilling is an option. There must be sufficient donor fat available, and the breast skin must be supple enough to accommodate the extra volume. In individuals with a very slender build, there is often insufficient fat to harvest for a meaningful result.  <\/p><h2 class=\"wp-block-heading\">How much volume gain is realistic with fat transfer<\/h2><p class=\"wp-block-paragraph\">This is the point where expectations and technique most often intersect. Not all transferred fat remains permanently. A portion is absorbed by the body in the first few months; this is called resorption. The fat that is still present after approximately six months is considered permanent.   <\/p><p class=\"wp-block-paragraph\">In practice, this means that one session of lipofilling usually provides a modest gain in volume. Those desiring more volume generally require multiple sessions, spaced several months apart. With implants, a larger and more predictable increase in volume can be achieved in a single procedure.  <\/p><p class=\"wp-block-paragraph\">Lipofilling is therefore often chosen by patients who desire a natural, subtle improvement, who do not want foreign material in their body, or who simultaneously prefer a refinement of the contour in the donor area.<\/p><h2 class=\"wp-block-heading\">Implants: material and placement<\/h2><p class=\"wp-block-paragraph\">Modern breast implants consist of a silicone shell with a cohesive silicone gel. They come in various volumes, widths, profiles, and shapes. The choice is made based on the width of the breast base, the thickness of the tissue covering the implant, skin quality, and the desired shape.  <\/p><p class=\"wp-block-paragraph\">Placement occurs in one of three positions:<\/p><ul class=\"wp-block-list\"><li><strong>Subglandular (over the muscle).<\/strong>  The implant is placed under the mammary gland, above the pectoralis major muscle. This provides direct shaping but requires sufficient natural tissue coverage to prevent the edges from becoming visible. <\/li><li><strong>Submuscular (behind the muscle).<\/strong>  The implant is placed entirely or largely under the pectoralis major muscle. The extra muscle layer masks the upper edge better and is an option for thin tissue coverage. The muscle may cause some movement when contracted.  <\/li><li><strong>Dual plane.<\/strong>  A combined technique: the upper pole of the implant lies behind the pectoral muscle, while the lower pole is in direct contact with the mammary gland. To achieve this, the lower edge of the muscle is partially detached. This technique combines a soft transition at the upper pole with good filling of the lower pole.  <\/li><\/ul><p class=\"wp-block-paragraph\">Which position is indicated depends on the anatomy and not on a prior preference.<\/p><h2 class=\"wp-block-heading\">Scars and recovery<\/h2><p class=\"wp-block-paragraph\">With lipofilling, scars are limited to a few entry points of a few millimeters. Recovery is primarily determined by the liposuction: bruising, swelling, and sensitivity in the donor area are normal and subside over several weeks. Compression garments are usually worn for several weeks.  <\/p><p class=\"wp-block-paragraph\">With <a href=\"https:\/\/www.drcolle.be\/en\/dienst\/breast-augmentation\/\">implants<\/a>, access is usually through a scar in the inframammary fold. During the first few days, there is tension and a feeling of pressure, especially with submuscular placement. Strenuous activities and sports are generally postponed for several weeks. The final shape only settles after a few months.   <\/p><h2 class=\"wp-block-heading\">Lifespan and follow-up<\/h2><p class=\"wp-block-paragraph\">A breast implant is not a lifelong device. The shell can weaken over time, capsular contracture (tightening of the capsule around the implant) may occur, or the shape of the breast may evolve due to pregnancy, weight fluctuations, or aging. Consequently, there is a chance that an <a href=\"https:\/\/www.drcolle.be\/en\/dienst\/implant-exchange\/\">implant exchange<\/a> may be necessary during one&#8217;s lifetime.  <\/p><p class=\"wp-block-paragraph\">Follow-up is therefore an integral part of the process. During imaging, it is important to inform the radiological team that implants are present. Lipofilling also requires correct imaging: transferred fat can cause small calcifications or fat cysts, which an experienced radiologist can usually distinguish well from other abnormalities. Participation in breast cancer screening remains indicated in both cases.   <\/p><h2 class=\"wp-block-heading\">The hybrid technique<\/h2><p class=\"wp-block-paragraph\">In some patients, both techniques are combined. The implant then provides the volume and projection, while lipofilling is applied in a thin layer over the upper pole and at the cleavage. This increases tissue coverage and softens the transition between the chest wall and the implant.  <\/p><p class=\"wp-block-paragraph\">This approach may be indicated for individuals with a thin build, pronounced asymmetry, or during an implant exchange where the tissue has become thinner over the years.<\/p><h2 class=\"wp-block-heading\">When a breast lift is part of the plan<\/h2><p class=\"wp-block-paragraph\">Volume and position are two different matters. When the nipple has clearly dropped relative to the inframammary fold, volume alone rarely provides the desired result. In that case, a <a href=\"https:\/\/www.drcolle.be\/en\/dienst\/breast-lift-mastopexy\/\">breast lift<\/a> is considered, possibly in combination with an augmentation. A lift repositions the nipple and firms the shape, but involves additional scarring.   <\/p><h2 class=\"wp-block-heading\">From consultation to decision<\/h2><p class=\"wp-block-paragraph\">During the first consultation, medical history, any family history of breast cancer, smoking habits, weight evolution, and the desire for children are discussed. A clinical examination follows, including measurements of the breast base, skin quality, and nipple position. <\/p><p class=\"wp-block-paragraph\">There is no single technique that is right for everyone. The choice is a balance between desired volume, available donor fat, acceptability of foreign material, scarring, and the willingness for long-term follow-up. Every procedure involves risks, including infection, postoperative bleeding, wound healing problems, changes in nipple sensation, and asymmetry. The result varies from person to person.   <\/p><h2 class=\"wp-block-heading\">Frequently asked questions<\/h2><details class=\"wp-block-details is-layout-flow wp-block-details-is-layout-flow\"><summary>Is breast augmentation with fat transfer permanent?<\/summary><p class=\"wp-block-paragraph\">The fat that remains after approximately six months is considered permanent. It is living tissue, meaning it fluctuates with weight changes. However, a portion of the transferred fat is absorbed during the first few months.  <\/p><\/details><details class=\"wp-block-details is-layout-flow wp-block-details-is-layout-flow\"><summary>How many lipofilling sessions are required?<\/summary><p class=\"wp-block-paragraph\">This depends on the desired volume, the available amount of donor fat, and the suppleness of the breast skin.
